You and your dependents are eligible to enroll for coverage.
If you decide after the policy takes effect to enroll additional dependents for coverage, you will need to provide MetLife with advanced written notice and any required premium to add the dependent(s). After we receive your written notice to add dependents, your premium rates will be adjusted as of the date coverage takes effect for the newly added dependent(s). The effective date of coverage for newly added dependent(s) will depend on when we receive a notice and the required premium.

When would my coverage start?
If you enroll by the 20th of the month, your coverage will be effective the 1st of the following month (i.e. enrolls on 9/14, effective 10/1).
If you enroll from the 21st to end of month, your coverage will be effective the first of the next month (i.e. enrolls 9/21, effective 11/1).

MetLife offers three Dental programs that you can choose from:
The Preferred Provider Organization (PPO) program is an individual dental insurance policy offered nationwide. The Dental HMO/Managed Care program is available to residents of California, Florida, New York and Texas. The MetLife Discount Dental program is not insurance, but provides you with discounted prices on a wide range of dental care services and products.

2The MetLife Discount Dental Program IS NOT INSURANCE and is not intended to replace health insurance. This plan does not meet the minimum creditable coverage requirements under M.G.L. c.111M and 956 CMR 5.00. This plan is not a Qualified Health Plan under the Affordable Care Act. The range of discounts will vary depending on the type of provider and service. The plan does not pay providers directly. Plan members must pay for all services but will receive a discount from participating providers. The list of participating providers is available on this site. A written list of participating providers is available upon request. You may cancel within the first 30 days after effective date or receipt of membership materials (whichever is later) and receive a full refund.
